Project Out

Partnerships to Independence

Project Out is designed to assist individuals who are in a nursing facility and want to return to independent living within the community/or those individuals who are in imminent danger of going into a nursing facility.

Project Out will ensure that services and supports are provided to maintain the highest quality of life for individuals returning to the community or individuals who wish to remain in the community. po1

Project Out IL Transition Specialists will work with a team of professionals to assess eligibility of individuals for consideration to return or remain independently in the community.


Transition Eligibility

1. Medicaid eligible and an LT 101 score of 13 pts. or more.

2. Individual of any age.

3. Disability or long term illness .

4. Wyoming Resident.


Diversion Eligibility

1. Medicaid eligible and an LT 101 score of 13 pts. or more.

2. Individual of any age.

3. Disability or long term illness.

4. Wyoming Resident.

5. Certificate of Eligibility signed by a medical professional stating that the individual is in imminent danger of institutionalization.


Services Provided

Housing assistance which may include:

First and last months rent.

Utility deposits.

Acquiring furniture.

Accessibility.

Adaptive equipment .

Application assistance.

Transportation assistance (thru our Transportation Voucher Program.)
