Community Choice Waiver Case Manager

(Part-Time - Sheridan - open until filled)

The work of Wyoming Independent Living is grounded on the Independent Living (IL) philosophy which maintains that persons with disabilities are the best judges of their service needs and how to receive those services.  Services are intended to assist consumers to attain the highest degree of independence and self-reliance possible. 

All WIL Program Specialists are responsible for providing the five core services of a Center for Independent Living:

  • Information and referral (I & R)
  • Independent living skills training
  • Personal and systems advocacy
  • Peer support, and
  • Transition services to youth and adults

The CCW Program Specialist works with individuals on the Community Choices Medicaid Waiver program. Specialist coordinates in-home services and supports and provides employer training so that consumers can effectively hire, train and supervise their own self-help assistants. Position involves extensive travel throughout the service area.

SPECIFIC DUTIES INCLUDE:

  • Performing comprehensive consumer interviews to establish goals for independent living
  • Ensuring compliance with program rules and regulations
  • Maintaining accurate and timely consumer records
  • Preparing reports and completing agency paperwork within established deadlines
  • Conducting community outreach and education activities
  • Promoting interagency collaboration to improve service delivery systems for people with disabilities
  • Travel throughout designated service area to meet with consumers and other stakeholders
  • Participate in staff training and related conferences/conventions
  • Other duties as assigned

QUALIFICATIONS

  • Bachelor Degree in Social Services or related field required
  • Experience at a Center for Independent Living preferred
  • Personal experience with a disability is preferred
  • Proficiency in Microsoft office applications

Driver / Assistant

(Part-Time - Casper- open until filled)

 

Wyoming Independent Living is currently seeking a driver based in Casper. This is a part-time position with a variable schedule from 15 to 20 hours per week. Additional hours may be scheduled as needed.

 The principal responsibilities for this position include:

  • Drives staff to scheduled appointments
  • Provides friendly, prompt, door-to-door, round trip service.
  • Provides clerical and administrative support to staff including marking appliances, reading, completing paperwork, filing, and other tasks as assigned.

Applicants should meet the following requirements:

  • High school diploma, GED, or equivalent preferred.
  • Flexibility in scheduling of hours for driving. Able to provide a clean motor vehicle in working order, with sufficient room for transporting staff and files or equipment. Proof of current registration and insurance required. Mileage is reimbursed at the approved federal rate.
  • Valid driver’s license with no more than two moving violations and/or at-fault accidents on driving record in the past three years.
  • Must complete pre-employment testing with results satisfactory to WIL, including but not limited to criminal history, motor vehicle records, drug test and sex offender registry.
  • Must have knowledge of relevant traffic laws and safe motor vehicle operation.
  • Must have the ability to read, understand, and follow route directions and maps.
  • Must have a positive attitude.
